The research team of Michal Eger, Nir Sterer, Tamar Liron, David Kohavi and Yankel Gabet from Tel Aviv University Israel, in a recent study, show that ultrasonic scaling around dental Implant made of titanium can aggravate Peri-implantitis.
Peri-implantitis is considered to be one of the many causes of long term implant failures. The loss of bone around implants is ususally the result of inflammation orchestrated by bacteria.
Treatment of Peri-implantitis typically begins with the use of ultrasonic scaler to clean the contaminated surface of the implants. The authors observed that the procedure of scaling causes release of ti particles into the surrounding micro environment which leads of 'aseptic inflammation' , a phenomenon known to be the primary casue of long term joint loosening after orthopedic joint replacement.
The in vivo aspect of the study found that particles released during ultrasonic scaling ‘stimulated the secretion of cytokines’.
In addition to this, researchers observed that the surface treatment of titianium implants through sand blasting and acid etching further complicated the situation. As it was the particles from sandblasted surfaces that caused the most inflammation.
The study was published in Scientific Reports titled "Scaling of titanium implants entrains inflammation-induced osteolysis."
